This document contains information regarding your rights and obligations in connection with your treatment by the Practitioner at the Lung and Sleep and the Practitioner’s billing procedure. It is important that you review this document before your first appointment to ensure that you are fully prepared and aware of the Practitioner’s procedures. If you proceed with your consultation, this document together with all other documents provided to you will comprise the agreement between you and the Practitioner.

Your initial appointment at our clinic: 

It is important that you provide the Practitioner with all relevant information at or before your initial consultation so that the Practitioner can provide you with the most complete and comprehensive consultation and treatment. Prior to the consultation, Lung and Sleep, on behalf of the Practitioner, will make efforts to retrieve any omitted information during the appointment,  however this is dependent on the fulfilment of requests by third parties.
You agree to provide the Practitioner with the information in a timely manner prior to the initial appointment and warrant that the information is accurate. You acknowledge that the medical advice and conclusions you received are based upon this information and other matters discussed during your consultation.

You acknowledge that further consultation(s) may be required in the event that all the required information is not provided at or before your initial consultation and an additional fee will be charged by the Practitioner for any additional consultation(s). If you have any queries regarding the information to provide, please contact the Practitioner or Lung and Sleep prior to your consultation.
You also understand that depending on your specific circumstances, upon completion of your initial assessment and consultation by the Practitioner, additional consultations may also be required.

Invoices and billing practices:

Fees and costs: The practitioners at the Lung and Sleep charge fixed fees for initial and any subsequent consultations required. These fees are set out at the time of booking your appointment.

We are a private clinic and fees charged by the Practitioner operating from the practice are not bulk billed or discounted unless otherwise advised in writing.

Additional fees: The Practitioner may also charge fees for any additional consultations required by the Practitioner at Lung and Sleep if requested by you. You will be informed by the Practitioner prior to the commencement of such additional consultation or attendances of the costs associated. These additional attendances/costs include but are not limited to:

  • completion of any additional forms/reports not included in the Practitioner’s engagement including
    completion of third party documents and letters
  • retrieval of any stored documents held by the Practitioner or the Clinic on behalf of the
    Practitioner.

Invoicing: At times the Clinic issues invoices on behalf of practitioners in which case the invoices will be sent to you by email to the address specified by you. If you would prefer that you receive such correspondence by mail or fax, please let the Practitioner or the Clinic know immediately.

GST: unless otherwise stated, the Practitioner’s fees are exclusive of GST.

Cancellation and non-attendance policy

Lung and Sleep and Lung Life Physiotherapy are always striving to provide quality care to our community
in a timely fashion. We have high demand for our services and we are very grateful that you choose us to
help you for your respiratory and sleep health. We try hard to maximise our efficiency to provide you with
timely care. To help us do this we must avoid late cancellations and non-attendance for appointments. We
will provide options for appointment reminders, so we feel it is reasonable to ask for adequate notice if you
need to change your appointment. As caring healthcare professionals, we are always striving to help as
many people as possible. To help achieve this, we have the following policies:

Appointment Reminders: We want to give you plenty of time to change or cancel your appointment if
needed.

  • Appointment reminders will be sent 7 days prior to your appointment via SMS.
  • If we don’t hear from you within 24 hours, we will call you.
  • If we can’t contact you, we will also send you a reminder email with the details of your
    appointment and a request that you call us to confirm your appointment.

Cancellation and Non-attendance: We require greater than 24 hours notice to reschedule or cancel an
appointment.

  • We will be able to offer an open appointment to another patient sooner, providing them with the
    care they need.
  • You can reschedule your appointment by calling Lung and Sleep on 03 9570 3893. If you can not
    reach us, please leave a message and we will call you back.
  • If you reschedule or cancel within 24 hours of your appointment or fail to attend your
    appointment, you will be charged 50% of the scheduled private fee for your consultation
  • You will be unable to use any medicare, private health insurance, EPC or DVA. contributions
    towards this fee
Engagement of another specialist or practitioner: 

The Practitioner may, in the provision of medical services to you, deem it necessary to engage with or liaise with other specialists or medical practitioners. The Practitioner will consult with you about the terms of these engagements if and when they believe they become necessary and before incurring any expense.

Your obligations: 

The Practitioner requires you to, and you agree to:

  • provide full and honest instructions relevant and any material change in your circumstances; and
  • co-operate and do all that the Practitioner reasonably requests of you in a timely manner.

If you fail to comply with any of these conditions, the Practitioner has the option to terminate its arrangements with you.

Privacy of personal information: 

Each Practitioner is committed to ensuring the privacy and confidentiality of your personal information. The Practitioner must comply with the Privacy Act 1988 (Commonwealth) and the Australian Privacy Principles and any other applicable privacy laws that govern how private sector Health Service Providers handle your personal information (inclusive of Sensitive Information and Health Information). Please review the accompanying privacy policy for more information. The disclosure of personal information is sometimes required and may be disclosed in accordance with the accompanying privacy policy. Other than in the course of providing medical services or as otherwise described in the privacy policy, the Practitioner will not share personal information with any third party without your consent. In most instances the Practitioner will require your consent prior to any disclosure. This includes the disclosure of any personal information regarding a third party (such as your spouse or family member) which you have provided to the Practitioner.

Internet communications: 

Your practitioner may communicate with you electronically. Electronically transmitted information cannot be guaranteed to be secure, error or virus free and it can be adversely affected or unsafe to use.
The Practitioner will not be liable to you in respect of any loss, damage, error or omission arising from or in connection with the electronic communication of information to you. Please let the Practitioner know if you do not wish to be communicated with by electronic means.
As you can appreciate however, the Practitioner will require face to face consultations to discuss your matters and the Practitioner will not undertake phone consultations unless determined necessary by the Practitioner.
With respect to the supply of test results, the Practitioner may supply these results electronically if you consent to the same. However, you acknowledge that the Practitioner may determine it necessary to coordinate a subsequent consultation to discuss these test results and the Practitioner reserves the right to charge for the consultation.

Definitions: 

In this document:

  • Practitioner means the individual medical practitioner who utilises our services in the carrying on of their medical practice and that practitioner who has been engaged by you in the provision of medical services.
